What is included
Pre-conference registration includes workshop materials. Full conference registration includes conference materials, continental breakfast, lunch on Friday, and continental breakfast on Saturday.

Refund policy
Full refunds of conference fees, less a $15 processing fee, will be granted if WRITTEN notice of cancellation is received by MCSHC’s conference coordinator, Steiner and Associates, with a postmark NO later than November 6, 2009. No refunds will be given after November 6, 2009.
However, substitutions are encouraged and are permitted. To authorize a substitute in your place, you will need to notify MCSHC's conference coordinator, Steiner and Associates, prior to the conference date. All cancellations and substitutions notifications must be submitted in writing.
Should the conference be canceled due to inclement weather, full refunds will be given. Should registrants need to cancel due to inclement weather, but the conference has not been canceled, with written notification received by December 31, 2009, they will receive a voucher for a 50% credit of what was paid, to be used for the 2010 Conference. If the weather is questionable, please call MCSHC’s conference coordinator, Steiner and Associates, at 573-489-0622 or check the MCSHC’s web site at www.healthykidsmo.org.
How to Register
Registrations can be made via online, mail, phone, or fax. Purchase orders are not accepted. No matter which registration method used, in order to be considered for the early bird discount fee, payment MUST be received by MCSHC before the November 7, 2009.
Online: Participants are strongly encouraged to use MCSHC’s online system for registration. The MCSHC online system will allow the participant to pay immediately by credit card or Paypal, or to be billed by email. If the participant chooses to be billed, he or she will receive an email invoice with instructions for check payment. Your registration is not complete until payment is received in full. All checks must be received by November 7th for the early bird registration fee.
Mail: If you are not able register online, you may complete the registration form and mail it with payment to Missouri Coordinated School Health Coalition (MCSHC), c/o Steiner and Associates, 501 S. Cedar Lake Drive, Columbia, MO, 65203.
Phone/Fax:Individuals can register by fax at 573-875-8158 or they may register by phone by calling MCSHC's conference coordinator, Steiner and Associates, at 573-442-2963 or 573-489-0622.
Purchase orders are not accepted. If you register by mail, phone, or fax, your registration is not complete until payment is received in full. Duplicate receipts will be issued until December 31, 2009, for a fee of $3.00 per receipt.
